Pathology

Exclusively a human-based, non-motile gram-negative set of rod-shaped bacteria.

Aetiology

There are four types of Shigella species ordered in terms of severity, ranging from mildest to most severe: Shigella dysenteriae, S. flexneri, S. boydii, S. sonnei.

Incubation

The usual range for incubation of Shigella is anywhere from one to seven days.

Symptoms

  • Headaches and abdominal pain (severe prodrome)
  • Diarrhoea

Signs

  • Fever
  • Dehydration

Investigations

Microbiology

Stool cultures are the primary method of investigation.

Treatment

Conservative

Generally will resolve on its own without clinical intervention.

Medical

Antibiotics may help reduce the severity, time course and infectivity by approximately two days.

Complications

  • Rectal prolapse
  • Haemolytic uremic syndrome (HUS)
  • Seizures in small children
  • Reactive arthritis

Prognosis

Excellent, though faecal carriage might continue for three to four weeks after the infection, and abdominal symptoms may persist for four to five weeks.
